The problem I have with Skype is that no matter what changes I make I cannot achieve a base-level sound quality that is dependable enough to use it on a regular basis.

Glad Skype is doing this. I have been using Tokbox www.tokbox.com for about a year now. With more than a few people on a call we will use a free conference phone as the video and audio together can cream the bandwidth.

For the record, I have it on good authority (OK, the horse's mouth) that judges in Connecticut have been seriously looking into video-conferencing for precisely this purpose.
